在申请 Occupational Therapy Doctoral Program，申请人必须向3个不同的地点提供资料:

• OTCAS
• The Occupational Therapy Department 和

3. Application 和 Personal Statement

4. Three (3) Letters of Recommendation

6. 必备的课程  

所有的 required prerequisite courses must be completed with a "B" or better. 所有的先决条件课程必须在进入程序之前完成. 至少 8门课程中的4门必须在提交申请时完成.
